You can save time before your first office visit by printing, filling out and signing the required forms on this page. The Therapist-Client Services Agreement discusses information concerning the practice, financial arrangements, confidentiality and patients’ rights. The Intake Form provides contact and insurance provider information. And the North Carolina Notice explains the policies and practices used to protect the privacy of your health information. The optional Consent Form is to be filled out only if you want us to communicate with another care provider or professional, such as your medical doctor, prior therapist, or psychiatrist.

If you are coming in for a psychoeducational or psychological evaluation, complete th
e Evaluation Services Form in addition to the forms above.

If you are coming for a custody or parenting plan consultation, please complete th
e Custody Services Form.

These forms are important informational documents and are for you to keep. You will need to bring the signature page of
the Therapist-Client Services Agreement and the Intake Form to your first appointment. We will be discussing these documents with you during your first visit, and will be happy to answer any questions you have at that time.
